Cirugía cardiovascular en España en el año 2004. Registro de intervenciones de la Sociedad Española de Cirugía Torácica y Cardiovascular

El registro de intervenciones de la Sociedad Española de Cirugía Torácica y Cardiovascular del año 2004 recoge los datos de 50 hospitales. Se intervinieron un total de 30.208 pacientes, con una media de 604 intervenciones/centro. Dieciocho mil seiscientas veinte fueron consideradas intervenciones mayores, ya fuese bajo circulación extracorpórea (CEC), revascularización coronaria u otras intervenciones importantes sin CEC, o cirugía congénita también sin CEC. La media de intervenciones por hospital fue de 372. Por tipos de intervención se dividen en: cirugía coronaria aislada 34,8%, cirugía valvular 38,7%, cirugía combinada 8,5%, cirugía de las cardiopatías congénitas 8,4% y misceláneas 9,5%. El número de intervenciones valvulares (7.207) es superior al de las revascularizaciones coronarias realizadas (6.487). La mortalidad global para la cirugía de revascularización coronaria aislada con CEC fue del 3,7% y sin CEC 3,2%. La media de injertos/paciente fue de 2,9 y 2,2, respectivamente. La mortalidad hospitalaria de los pacientes intervenidos con CEC, según el número de injertos/paciente, fue de 3,3, 3,5, 3,8, 3,7 y 4,5% para uno, dos, tres, cuatro y cinco o más injertos. Se implantaron un total de 10.298 prótesis valvulares, de las que el 56% eran mecánicas. De un total de 8.792 intervenciones de cirugía valvular, en 6.838 se actuó sobre una válvula, y de ellos en 4.721 (69%) fue sobre la válvula aórtica, con una mortalidad del 5,4 y 9,1% si se asoció revascularización. Se realizaron 2.065 intervenciones sobre la válvula mitral aislada, con una mortalidad del 7,5 y 11,2% si además hubo revascularización. Se practicaron 1.287 intervenciones sobre dos válvulas y 227 sobre tres válvulas, con una mortalidad del 8,6 y 13,6%, respectivamente. Se intervinieron 1.560 pacientes para corrección de sus cardiopatías congénitas, de éstos 1.151 fueron procedimientos con CEC y 409 sin ella. Se realizaron 256 trasplantes cardíacos en adultos, con una mortalidad hospitalaria del 12,9%. Se llevaron a cabo 4.159 implantaciones de marcapasos, 1.469 cambios de generador, 430 implantaciones de desfibriladores y 202 cambios de generador desfibrilador.Se intervinieron 3.935 pacientes de cirugía vascular periférica. Mil ciento noventa enfermos fueron intervenidos por aneurisma aórtico de alguna localización, de los que 945 (80%) necesitaron CEC para realizar la corrección.

The Spanish Society of Thoracic and Cardiovascular Surgery registry for the year 2004 includes data from 50 hospitals. Within this year a total of 30,208 patients were operated on, with an average of 604 operations/center. With extracorporeal circulation, aortocoronary bypass off-pump and congenital cardiac surgery were operated 18,620 patients with an average of 372 cases/hospital. Interventions were 34.8% coronary surgery, valvular surgery 38.7%, combined surgery (valvular and coronary) 8.5%, congenital heart surgery 8.4% and 9.5% miscellaneous. The number of valvular surgeries was superior to that coronary procedures (7,207 vs. 6,487) and combinated 1,585. Total hospital mortality was 3.7% for the coronary revascularization with extracorporeal circulation and 3.2% in off-pump. The average of coronary graft/patient was 2,9 with ECC and 2.2 without ECC. The hospital mortality for revascularization with extracorporeal circulation was 3.3, 3.5, 3.8, 3.7 and 4.5% for one, two, three, four or five or more grafts.The number of valvular prosthesis implanted was 10,298 and the 56% of these are mechanical. From the total number of 8,792 patients with valvular surgery, 6,838 underwent one valve surgery. Aortic valve was operated in 4,721 (69%) patients with a hospital mortality of 5.4 and 9.1% if associated with coronary revascularization. Mitral valve surgery was done in 2,065 patients and with hospital mortality of 7.5 and 11.2% in cases combined with revascularization. Two valve surgery was done in 1,287 patients and 227 patients was three valve surgery and hospital mortality was 8.6 and 13.6%, respectively.The number of patients operated on for congenital cardiac defects was 1,560 and there were 1,151 open heart surgeries and 409 closed. Heart transplant were done in 256 adults patients with hospital mortality of 12.9%. Pacemakers were implanted in 4,159 patients and pacemaker generator exchange was done in 1,469. Implantation of defibrillator in 430 cases and defibrillator exchange in 202 patients.There were 3,935 operations by peripheral vascular surgery. The number of aortic aneurysms operated on during this year was 1,190 for all localizations and them 945 80% was done with extracorporeal circulation.
