Abstract
Glucose metabolism alterations (impaired fasting blood glucose, impaired glucose tolerance and diabetes mellitus) are one of the major health problems with an increasing incidence and prevalence. A cluster of risk factors (arterial hypertension, dyslipemia, obesity, etc) and other cardiovascular risk determinants (higher inflammation mediators, increased trombogenicity, impaired fibrinolysis, etc) are associated with these glycemic alterations. All these facts are included in the metabolic syndrome with an increased insulin resistance as the mediator. Isolated glucose control has a limited impact on cardiovascular protection; an integrated risk factors correction has a major role in limiting vascular and renal damage. In this sense, new data on drugs that improve insulin resistance suggests beneficial effects on diabetes incidence and vascular damage.
