Guidelines (recommendations) created by scientific societies or panels of experts do not have the strength of regulations, but as they rely on current evidence based medicine, they are the source of important medical knowledge in every day practice. Guidelines should be discriminated from standards, which are legal regulations being an area of responsibility of ministry of health. Allergen immunotherapy is one of the fundamental procedures in allergology.Current national and international guidelines critically review and up-date the knowledge addressed to allergen immunotherapy. For different reasons AIT is especially beneficial for children. In the European Union registration of allergen products for both diagnostic and treatment purposes is regulated by certain directives and is subject to supervision of European Medical Agency. For the purpose of performing high quality clinical trials with allergen immunotherapy in children, pediatric investigational plan was introduced. Its high requirements create a high-risk situation which may limit introduction of AIT in pediatric population. It arises concern of medical authorities.
