Fasciite nécrosante compliquant un bandage de Dujarier : à propos d’une observation

RésuméLa dermohypodermite bactérienne nécrosante avec fasciite nécrosante (DHBN-FN) désigne une infection grave, de l’hypoderme, de l’aponévrose superficielle et secondairement du derme. Nous rapportons le cas d’une patiente présentant une dermohypodermite nécrosante avec fasciite de la paroi thoracoabdominale compliquant un bandage de Dujarier. Il s’agissait d’une DHBN-FN monobactérienne à streptocoques β hémolytiques, dont l’évolution a été favorable grâce à une prise en charge médicochirurgicale adaptée. L’hypothèse évoquée est la survenue d’une infection secondaire des nécroses cutanées apparues sous le bandage de Dujarier trop serré en regard de la zone d’appui du coude sur le tronc. Nous rappelons, à travers ce cas, l’importance de la prise en charge médicochirurgicale et aussi l’importance de la confection de tout bandage thoracoabdominal, pouvant conduire, s’il est serré, à une escarre pouvant constituer une porte d’entrée pour toute DHBN-FN avec en plus un retard diagnostic.

SummaryThe necrotizing fasciitis (NF) means a serious infection of the subcutis, the fascia and superficial dermis secondarily. We report the case of a patient with cellulitis NF of the thoracoabdominal wall complicating Dujarier's bandage. It was monobacterial NF caused by β hemolytic streptococcus, whose development was supported through an appropriate medical and surgical care. The situation mentioned is the occurrence of secondary infection of skin necrosis occurred in the bandage too tight opposite the support zone of the elbow on the trunk. We point out, through this case, the importance of the medicosurgical and also the importance of making any thoracoabdominal bandage, which could lead, if it is tight, a pressure sore can be a door entry for any NF plus a late diagnosis.
