Évolution fatale après réactivation d’une hépatite chronique virale B occulte dans les suites d’une chimiothérapie pour lymphome chez un malade porteur de l’anticorps anti-HBc isolé

RésuméLa réactivation d’une hépatite chronique virale B à l’arrêt de la chimiothérapie chez un porteur de l’antigène HBs est un fait connu et il est recommandé de prescrire un traitement antiviral avant de débuter une chimiothérapie chez ces malades. Une réactivation B chez les malades ayant les anticorps anti-HBs et anti-HBc positifs est rare mais néanmoins possible. La survenue d’une réactivation B chez un malade porteur de l’anticorps anti-HBc sans virémie est exceptionnelle. Nous rapportons le cas d’une femme suivie pour un lymphome B non hodgkinien de haut grade de malignité traité par 6 cures de chimiothérapie de type CHOP (cyclophosphamide, adriamycine, vincristine, prednisone) et rituximab. Le bilan préthérapeutique trouvait un anticorps anti-HBc positif isolément avec un ADN viral B indétectable. Deux semaines après l’arrêt de la chimiothérapie apparaissait une élévation des transaminases avec une charge virale B supérieure à 100 000 000 copies/mL. Les autres causes d’élévation des transaminases étaient éliminées et le diagnostic de réactivation virale B était posé. Malgré l’introduction précoce d’un traitement par lamivudine et adéfovir, la malade décédait d’une hépatite fulminante. Seuls 13 cas de réactivation virale B après chimiothérapie chez des malades présentant un anticorps anti-HBc positif ont été rapportés dans la littérature.

SummaryHepatitis B reactivation after chemotherapy is well known when Ag HBs is positive. Recommendation is to give preventive antiviral treatment before starting chemotherapy. The reactivation when there is only an anti-HBc antibody is rare. We report the case of a woman who developed an hepatitis B reactivation two weeks after the end of a chemotherapy for a high grade non Hodgkin lymphoma. Before chemotherapy, hepatitis B virus serology was positive only for anti-HBc antibody and viral load was negative. She had a fulminant hepatitis with fatale issue although a treatment by lamivudine and adefovir was rapidly started. In the literature, only 13 cases of patients with positive anti body anti-HBc alone who developed an hepatitis B reactivation after chemotherapy have been reported.
