Abstract
												Community-hospital coordination must be an objective shared by all health care professionals involved in the treatment of cancer. It requires a certain level of collaboration between all protagonists, i.e. the patient, the caregivers and the professionals, from diagnosis through to the post-cancer period. It must consider patients as a whole, meaning their physical condition, before, during and after the disease, their mental state and their socio-economic situation.
