Abstract
Patellofemoral instability is a common problem of the knee. It can occur as a first-time dislocation or as chronic instability. The most common dislocation mechanism is a near extension load with a valgus moment and can be found in several sports. Ligamentous, bony, and neuromuscular factors influence patella stability. A targeted analysis of the injury and risk factors can be realized by a structural examination and serves as a basis for differentiated treatment. In the case of minor risk factors, conservative treatment can be carried out which includes specific treatment options. When major risk factors or chondral leasions exist surgical measures may be necessary. The basic treatment is MPFL suture or reconstruction. According to further factors, additional interventions, such as corrective osteotomies or a trochleaplasty, could be indicated or represent the main intervention.
