Abstract
The case is presented of an 82 year old patient with abscessed pneumonia in the left lower lobe, associated with ipsilateral pleural effusion of fungal origin (Candida albicans), and with septic shock and ventilatory failure. After studying the imaging findings, decortication, removal of adhesions, pleural fluid drainage, and closed tube thoracotomy drainage was performed. Cytochemistry of the pleural fluid showed that it was compatible with the characteristics of enteral nutrition. Upper digestive tract endoscopy, after testing positive for methylene blue, showed a mucosal tear at the level of the gastro-oesophageal junction, thus a gastric stent was inserted. Due to the poor progress of patient, persistent purulent drainage, impaired breathing pattern, the patient died.
