Antibioprophylaxie pour césarienne : modification des pratiques et administration avant incision, enquête de faisabilité auprès des pédiatres du réseau de santé périnatale d’Auvergne (RSPA)

RésuméObjectifDécrire les connaissances des pédiatres concernant la pratique de l’antibioprophylaxie pour césarienne en référence à la Conférence de consensus de la Société française d’anesthésie réanimation (Sfar) et évaluer la faisabilité d’un changement de pratique (injection de l’antibiotique avant incision), auprès des pédiatres du réseau de santé périnatale d’Auvergne (RSPA) exerçant en maternité.Type d’étudeEnquête de type transversale effectuée par questionnaire.MéthodologieEnvoi d’un premier questionnaire aux 46 pédiatres du RSPA exerçant en maternité. Presque un tiers des pédiatres ayant renvoyé ce questionnaire se disaient non concernés. Un second questionnaire a donc été élaboré en collaboration avec deux pédiatres du CHU de Clermont-Ferrand puis envoyé aux mêmes 46 pédiatres. La partie statistique a comporté le calcul de pourcentages.RésultatsLes taux de réponse ont été relativement satisfaisants avec respectivement 61 % et 67 %. Lors du premier questionnaire, seulement 25 % des pédiatres connaissaient l’antibiotique administré et le moment de l’injection. Sur le second questionnaire, 87 % étaient favorables à une administration avant incision et 42 % pensaient qu’elle serait sans effet sur la prise en charge du nouveau-né. Pour 35 % des répondants, elle pourrait entraîner une modification de la durée de l’antibiothérapie en cas d’infection materno-fœtale et pour seulement 13 % un retard à la mise en œuvre d’une antibiothérapie chez l’enfant.ConclusionLes pédiatres du RSPA ne connaissaient pas les pratiques de l’antibioprophylaxie pour césarienne. Ils n’apparaissent cependant pas opposés à une administration avant clampage du cordon puisqu’elle n’entraînerait pas de retard à la mise en œuvre d’une antibiothérapie éventuelle chez le nouveau-né.

ObjectiveTo describe the knowledge of paediatricians regarding the practice of antimicrobial prophylaxis for caesarean section in reference to the Consensus Conference of the French Society of Anesthesia and Intensive Care (SFAR) and assess the feasibility of a change in attitude (injection of the antibiotic prior to incision) among paediatricians Perinatal Health Network of Auvergne (RSPA) working in maternity.Study designCross sectional study by survey.MethodsFirst questionnaire was sent to 46 RSPA paediatricians working in maternity. Almost one-third of paediatricians who returned the questionnaire said they were not concerned. A second questionnaire was developed with two paediatricians of the CHU and sent to the same 46 paediatricians. The statistical part involved percentages.ResultsResponse rates were respectively 61% and 67%. For the first questionnaire, only 25% of the paediatricians knew the antibiotic and the time for injection. For the second questionnaire, 87% were in favour of an administration before incision and 42% thought it will not affect the care of the newborn. For 35% of respondents, it could lead to a change in the duration of antibiotic therapy in cases of perinatal infection and for 13% only a delay in the implementation of antibiotic therapy in children.ConclusionThe RSPA paediatricians did not know the practices of antibiotic prophylaxis for caesarean section. However, they did not appear opposed to an administration before cord clamping as it would not delay the implementation of any antibiotics in the newborn.
