RésuméLe syndrome des antiphospholipides (SAPL) associe des épisodes de thromboses vasculaires à la présence d’un anticorps antiphospholipide, et indique un traitement anticoagulant au long cours. Cette maladie peut évoluer vers un syndrome catastrophique des antiphospholipides (CAPS), véritable tableau de défaillance multiviscérale dont le taux de mortalité reste élevé. Nous présentons ici le cas d’une patiente porteuse d’un SAPL ayant présenté un CAPS après l’interruption de son traitement anticoagulant pour hémorragie digestive. Une prise en charge spécifique et agressive, associant corticothérapie, échanges plasmatiques et héparinothérapie curative, a permis une évolution favorable.
Antiphospholipid syndrome associate thromboembolic events (arterial or venous), and presence of antiphospholipid antibodies, and require anticoagulation. A catastrophic variant may develop, resulting in multiorgan failure, with high mortality rate. This article presented a patient with antiphospholipid syndrome presenting a catastrophic antiphospholipide syndrome after anticoagulation suspending for gastrointestinal bleeding. Multidisciplinary management in intensive care unit and aggressive therapies (corticosteroids, anticoagulation, plasma exchange) were essential to rescue the patient.
