Pourquoi une fiche d’information du patient après une intubation difficile ?

RésuméL’intubation difficile reste une source importante de morbidité, voire de mortalité en anesthésie. La moitié environ de ces intubations difficiles n’est pas prévue. Bien qu’une difficulté d’intubation soit reconnue comme un critère important d’intubation difficile lors d’anesthésies ultérieures, les enquêtes montrent que l’information est insuffisamment fournie au patient ou au médecin traitant. Différentes techniques de diffusion de l’information peuvent être proposées. La plus simple est probablement de fournir un document au patient et d’informer par écrit le médecin traitant. Nous proposons dans cet article un document type, respectant des critères retrouvés dans la littérature. Cela s’inscrit dans une démarche d’amélioration de la qualité dont l’objectif est de diminuer la morbidité liée à l’intubation difficile.

Unanticipated difficult tracheal intubation may be challenging to anaesthesiologists. It is still associated with morbidity or mortality. Previous difficult intubation is vital information to organize appropriate airway management. Unfortunately, previous studies in the literature have shown that there is poor communication of this information. We propose in this article an airway alert form, following several criteria published in the literature. This is, in our opinion, an important step to improve the efficiency of guidelines on airway management and the overall quality of the anaesthesia process. Its efficacy to decrease morbidity related to difficult tracheal intubation remains to be demonstrated.
