Myocardites

RésuméLa myocardite est une pathologie inflammatoire du myocarde, associée à une dysfonction cardiaque. Ses étiologies sont nombreuses, dominées par les causes virales, et ses présentations cliniques très diverses : simple augmentation des enzymes musculaires cardiaques lors d’une péricardite bénigne virale, myocardite fulminante avec état de choc cardiogénique ou parfois découverte devant une insuffisance cardiaque chronique. La reconnaissance des différents tableaux cliniques et échocardiographiques des myocardites est très utile pour la démarche diagnostique, la recherche d’une étiologie et adapter la prise en charge thérapeutique. Les progrès récents concernant les myocardites sont marqués par une meilleure analyse des mécanismes physiopathologiques, une approche diagnostique plus performante grâce à l’imagerie non invasive (IRM) et la biologie moléculaire. Mais le traitement spécifique reste à l’heure actuelle limité. Les essais thérapeutiques avec les antiviraux ne sont pas concluants, et la prise en charge repose donc essentiellement sur le traitement symptomatique de l’insuffisance cardiaque.

Myocarditis is an inflammatory disease of the myocardium associated with cardiac dysfunction. Etiologies of myocarditis are numerous – viral causes being the most frequent – as well as their clinical presentations which varies from isolated increase in cardiac enzymes during a viral pericarditis, fulminant myocarditis associated with cardiogenic shock to endomyocardial biopsy proven inflammation discovered during the etiologic diagnosis of a dilated cardiomyopathy. This article will discuss the importance of recognition of specific clinical scenarios of myocarditis and their echocardiographic presentations that are very useful for the etiologic diagnosis and to decide the medical strategy. Recent advances in the field of myocarditis concern improvement in understanding the pathophysiology, in the diagnostic approach with the use of noninvasive imaging (MRI) and molecular biology. However, specific treatment is still limited. Clinical trials with antiviral medications are not conclusive, and the medical strategies remain mainly based on the symptomatic treatment of heart failure.
