Verrucous carcinoma (VC) is a distinct form of oral carcinoma, which occurs mainly as an exophytic growth on the buccal mucosa and gingiva. VC that occurs as an intraosseous lesion of the jaws is extremely rare and very few cases about it have been reported until date. We present a rare case of VC of the maxilla, where the patient's chief complaint was a painless exophytic lesion in the upper left quadrant of the jaw over one year. On examination, it was observed that the lesion involved the underlying bone with expansion into the maxillary sinus. It was resected by partial maxillectomy. Histopathological examination of the excised specimen revealed a verrucous surface with parakeratinized clefts, bulbous rete ridges, and a pushing border with no invasion, which was consistent with the diagnosis of VC. Moreover, the patient had a history of an odontogenic cyst in the left maxillary region; therefore, we presumed that the present lesion probably originated from the previous cystic lesion.
