Fièvre prolongée et candidose hépatosplénique, efficacité de la corticothérapie

RésuméNous rapportons le cas d’un syndrome fébrile persistant au cours d’une candidose hépatosplénique (CHS) chez un patient traité pour un lymphome. L’inefficacité du traitement antifongique et la disparition spectaculaire des symptômes sous corticoïdes étayent l’hypothèse que nous discutons de la participation d’un syndrome inflammatoire de reconstitution immunitaire.

We report the case of a young male treated for lymphoma and presenting with hepatosplenic candidiasis when recovering from aplasia. Prolonged and adequate antifungal therapy failed to produce apyrexia, which was rapidly obtained with corticosteroid therapy suggesting the existence of an underlying immune reconstitution inflammatory syndrome.
