La scintigraphie hépatobiliaire : aspects et applications d’une technique sous-exploitée

RésuméNous présentons deux cas cliniques illustrant deux indications de la scintigraphie biliaire, volontairement très différents pour illustrer le spectre d’application de cette technique. Le premier cas rapporté est celui d’un jeune homme ayant bénéficié d’une transplantation hépatique auxiliaire pour hépatite fulminante et dont la scintigraphie biliaire a permis de suivre l’évolution de la fonction du foie natif et du greffon et de déterminer le moment opportun de diminution, puis d’arrêt du traitement antirejet. Le second cas est celui d’un homme d’âge moyen, rapportant des douleurs abdominales invalidantes et dont la scintigraphie biliaire a permis de les mettre en rapport avec une dysfonction du sphincter d’Oddi.

We present two clinical cases illustrating two indications of cholescintigraphy that are purposely very dissimilar to illustrate the panel of application of this technique. The first reported case is a young man who received an auxiliary-liver transplantation for the treatment of fulminant hepatitis, in whom cholescintigraphy enabled to follow the evolution of the graft and the native-liver function and to determine the appropriate time to (gradually) reduce and then cease the antirejection-drug treatment. The second case is a middle-aged man with incapacitating abdominal pain. Cholescintigraphy related pain to a dysfunction of the sphincter of Oddi.
