In the present report, we will make a review of the importance of the decisions making in the surgical treatment of the ischemic myocardiopathy. First, we will deal with the fundamentals of the indication of surgery, besides the importance of making it in a multidisciplinary way through the “Heart Team”. Second, we will help how to choose the correct moment of the surgical procedure, based on different factors. Finally, we will address the expectations of the surgical treatment from the point of view of the clinical cardiologist.
