Thrombophlébite cérébrale du sujet âgé

Cerebral venous thrombosis is an uncommon pathology in the elderly. We report the case of an autonomous 85-year-old patient presenting minor neurocognitive disorders, and addressed to emergency department for delirium. No clinical or biological triggering factor was found. After the brain scan, the diagnosis made was that of an intra-parenchymatous hematoma in the lower right parietal lobe with a suspicion of underlying brain tumor. A few days later, an injected MRI was performed. There was no sign of a tumor but venous thrombosis of the right lateral sinus was suspected. It was finally the injected brain scan that confirmed this diagnosis. A curative anticoagulation treatment was instated. The evolution of the situation was partly favorable with a decline in the confusion but persistence of marked temporospatial disorientation accompanied by moderately severe memory impairment, resulting in a state of dependence.
