Existe-t-il un rapport entre les incisions et la forme finale d’un sein ?

RésuméObjectifLes résultats des plasties mammaires – réduction, redrapage – revêtent une variété non négligeable de formes forcément tributaires des particularités de chacune des techniques utilisées. De façon plus précise, il nous est apparu que l’usage d’une cicatrice verticale anormalement longue privilégie une forme finale sous-optimale de sein à trop grande projection par rapport à sa hauteur. Devant l’enthousiasme de la clientèle pour des cicatrices réduites, on remarque dans la littérature une floraison de propositions de plasties à incision verticale unique, mais trop longue… La multiplication de ces publications nous expose à une constatation de plus en plus évidente et répétée : ces seins reconstruits avec une incision verticale trop longue ont tous le même défaut : sein en banane ou en forme de courge.MéthodeNous avons étudié la totalité des publications sur les mammoplasties à incision verticale des 15 dernières années et réalisé qu’elles partagent le même défaut : un rapport hauteur/projection anormalement bas. Ces chiffres ont été confrontés au rapport d’or du sein établi par le premier auteur (2005) [1].RésultatsLe rapport d’or du sein de Moufarrège privilégie un chiffre égal, proche ou supérieur à 2. Des résultats entre 2 et 1,5 sont jugés acceptables. Mais tout rapport hauteur/projection inférieur à 1,5 correspond à des formes de seins sous-optimales. Or, la totalité des résultats répertoriés dans la série étudiée des mammoplasties à cicatrice verticale avec une forme non harmonieuse correspondent à un rapport hauteur/projection inférieur à 1,5.ConclusionLes promoteurs de l’incision verticale ont le grand mérite de sensibiliser la population des chirurgiens plasticiens à l’avantage de l’abandon de la mutilante « ancre de bateau ». Cependant, ils devraient tenir compte de l’effet déformant de l’incision verticale exagérément longue et la raccourcir en la transformant en un T inversé à branches horizontales courtes, ce qui n’est pas payer trop cher pour obtenir un résultat esthétique plus attrayant.

SummaryGoalThe results of reduction mammoplasty and mastopexy demonstrate a great variety of shapes derived from the particularities inherent to each technique. More precisely, it has become apparent to us that the usage of an abnormally long vertical scar leads to a suboptimal final shape of the breast due to the excessive projection compared to its height. Although our clientele continually asks to minimize scars, we have noticed a resurgence in the literature of vertical incision mammoplasty techniques albeit with a scar of an excessive length. The multitude of publications leads to an evident and consistent conclusion: breasts reconstructed with an overly long vertical incision all suffer the same deformity characterized by a banana or squash-shaped breast.MethodsWe studied the totality of publications on vertical incision technique mammoplasty from the last 15 years and realised they all presented the same problem: an abnormally low height/projection ratio. These numbers have been confronted by the golden ratio established by the first author (2005) [1].ResultsThe breast's golden ratio developed by Moufarrège is optimal at a number close to, or superior to 2. Results between 2 and 1.5 are acceptable. All height/projection ratios inferior to 1.5 correspond to suboptimal breast shapes. Furthermore, the totality of results in the vertical scar mammoplasty technique which displayed unsatisfying results had a height/projection ratio inferior to 1.5.ConclusionThose who promote the vertical incision have the greatest drive to encourage other plastic surgeons to abandon the traditional mutilating boat anchor scar. Nevertheless, they should always remember the deforming effect caused by exaggerated vertical incisions which may be avoided by transforming it into an inverted T with short horizontal branches, a small price to pay to obtain a superior aesthetic result.
