Tratamiento de las taquicardias

ResumenEl objetivo del tratamiento de las taquicardias consiste en tratar el episodio agudo cuando el paciente esté en taquicardia sostenida, y a largo plazo prevenir y/o controlar nuevos episodios arrítmicos, así como evitar la muerte súbita en el caso de las arritmias ventriculares malignas. El manejo inicial dependerá de la clínica y tolerancia hemodinámica del paciente. En base a estos parámetros se decidirá finalizar la arritmia mediante choque externo o mediante fármacos. Una vez solventada la fase aguda, cada tipo de arritmia deberá ser evaluada para prevenir nuevos episodios. Este tratamiento crónico ha evolucionado de forma considerable en las últimas décadas, y la ablación por catéter ha sustituido a los fármacos antiarrítmicos como tratamiento definitivo de algunas arritmias. Por otro lado, el desfibrilador automático implantable (DAI) se ha convertido en la terapia estándar para pacientes con arritmias ventriculares malignas o alto riesgo de padecerlas.

Management of acute episode is the goal treatment in patients with sustained tachycardia. Long-term objetive is to prevent and/or control new arrhythmic episodes and avoiding sudden death in malignant ventricular arrhythmias. Patient's pattern of presentation and hemodynamic tolerance provide knowledge for the initial management. According to these data, decisions will be made to reverse the arrhythmia by external cardioversion or antiarrhythmic drugs. Once the acute phase has been solved, the arrhythmia should be evaluated to prevent further episodes. Long-term tachycardia management options have evolved considerably in recent decades, and catheter ablation has substitued antirrhytmic drugs as definitive treatment. On the other hand, implantable cardioverter-defibrillators (ICDs) are standard therapy in patients with a risk of sudden death due to a malignant ventricular arrhythmias.
