RésuméL'hémangioendothéliome épithélioïde est une tumeur d'origine vasculaire, rare, classée en potentiel d'agressivité entre les hémangiomes et les angiosarcomes. L'évolution reste principalement locale mais peut également donner des métastases. Nous rapportons ici le cas d'un patient atteint d'un hémangioendothéliome épithélioïde rachidien thoracique. Apres une première exérèse, une récidive locale a nécessité une reprise chirurgicale et une radiothérapie externe. Celle-ci a été effectuée avec modulation d'intensité. Au cours de la surveillance, une deuxième localisation rachidienne inopérable a nécessité une nouvelle séquence de radiothérapie effectuée avec modulation d'intensité avec boost intégré. À 24 mois du suivi, aucun signe de progression locale ou à distance n'a été noté chez notre patient. Cette observation est suivie d'une discussion avec revue de la littérature sur les autres cas d'hémangioendothéliome épithélioïde traités par irradiation.

Epithelioid hemangioendothelioma is a rare vascular tumour, with features between hemangiomas and angiosarcomas. Evolution remains mainly local but may also metastasize. Here, we report a case of a patient with a spinal epithelioid hemangioendothelioma. After a first resection, local recurrence required a new excision and external radiotherapy. It was achieved by helical TomoTherapy® with intensity modulation. During the follow-up, a second inoperable spinal location necessitated a new sequence of radiotherapy in modulation of intensity with integrated boost. The patient was alive at 24 months of follow-up without evidence of local relapse or distance. This observation is followed by a discussion with review of the literature on other epithelioid hemangioendothelioma cases treated with radiation.
