Dimensional correlates of poor insight in obsessive-compulsive disorder

► Poor insight in OCD was correlated with increased OCD severity and increased hoarding symptomatology. ► Other clinical variables were not correlated with level of insight in OCD, after OCD symptom severity was controlled for. ► Thus, insight represents a distinct characteristic entity associated with worse illness severity. ► Insight might be a moderator or mediator of treatment effects in OCD, especially in behavioral therapy. ► Further studies should examine the role of insight as a moderator and mediator of treatment response in OCD in both behavioral therapy and pharmacological trials.
