If a mold is isolated in a nail, it is considered of significance if appropriate fungal elements (mycelium, arthrospores, conidia) are seen on direct microscopy. At least 5/20 inocula must have yielded the mold. A second culture must have isolated the same mold. In addition, a nail biopsy must have visualised filaments in the nail plate. If a dermatophyte is associated, mold must not be neglected, because mixed infections are not exceptional.
