An ontology-based personalization of health-care knowledge to support clinical decisions for chronically ill patients

► An ontology for the health care of chronically ill patients at home is introduced. ► The ontology is extended with intervention plans for 19 chronic diseases. ► A method to personalize the ontology and the intervention plans is implemented. ► A MDSS uses the personalized ontology to help clinicians to make decisions. ► Tests confirm clinicians' satisfaction on these tools in controlled real settings.
