Syringomyelia is a disorder characterized by the presence of cavities within the spinal cord, most frequently located in the cervical and upper thoracic regions. The pathogenesis of this disease has not been fully elucidated. We present a case of a boy diagnosed at 2 years of age due to a psychomotor development delay. By physical examination on admission mild dysmorphic facial features, short neck and generalized hypotonia with presented deep tendon reflexes were observed. On the basis of magnetic resonance imaging extensive syringomyelia coexisting with the Chiari I malformation was diagnosed. In the described child posterior fossa decompression was performed with improvement in the form of significant regression of syringomyelic cavities.
