Blunt traumatic aortic rupture is a life-threatening emergency usually related to a violent crash involving sudden deceleration. Treatment remains controversial in surviving patients who reach the hospital. The traditional management has been repair of the injury as soon as possible. Although techniques in aortic surgery have improved over the years, morbidity and mortality rates still remain high and are often the result of another associated injury. Delayed repair of the acute aortic tear has been increasingly reported in selected cases, allowing the patient to recover from other major injuries. More recently, with the introduction of the endoluminal aortic stent grafting, a new strategy was introduced for treatment of acute traumatic aortic rupture.
