Anatomie du système lymphatique pelvien

RésuméLe système lymphatique pelvien draine la lymphe des viscères des appareils urinaire, génital et digestif du pelvis. Il est constitué de ganglions (nœuds) et de vaisseaux lymphatiques disposés dans le tissu conjonctif sous-péritonéal pelvien, au voisinage des viscères (ganglions viscéraux), mais surtout le long des vaisseaux iliaques externe, interne et commun (ganglions iliaques). Aux lymphocentres iliaques externes, internes et communs arrivent des vaisseaux afférents drainant les différents viscères pelviens. De ces lymphocentres s’échappent des vaisseaux efférents rejoignant les collecteurs lymphatiques sus-jacents qui aboutiront aux troncs lombaires. Étant la voie privilégiée d’essaimage des cellules cancéreuses, la connaissance des voies lymphatiques, de leur localisation et de leur drainage est indispensable dans un bilan carcinologique.

The lymphatic system of the pelvis collects the lymph of the genital and urinary organs and of the digestive tract. It is formed by lymphatic nodes and vessels situated inside the conjunctive tissue, near the organs (visceral lymphatic nodes) but especially along the external, internal and common iliac vessels (iliac lymphatic nodes). These nodes receive afferent vessels issued from the different pelvic organs. From the iliac lymphnodes arise efferent vessels running towards lymphatic collectors, situated above them, and which end in the lymphatic lombar duct. The lymphatic pathways represent the preferential way of scattering of cancerous cells. Therefore, the knowledge of the anatomy, of the situation and of the draining of the nodes is of the utmost importance in the evaluation of a cancer of a pelvic organ.
