Non-heartbeating organ donation has regained interest in France because of the shortage of grafts. The quality of the renal grafts from non-heartbeating patients is now comparable to that of brain-death donors offering a new hope to patients awaiting transplantation. The out-of-hospital organization, first link of this strategy, is very important for the success of this technique. Accurate selection of the donor and respect of ischemia times are mandatory to ensure the quality of the transplantation. Clear and honest information to the families during the prehospital phase must be prepared and adapted to the various situations. This technique raises also ethical questions that must be discussed.
