The US health system for organizing and delivering healthcare services is under tremendous pressure for change driven by both the market and the policy sector.1 The challenges to lower costs, improve quality, expand access, and enhance the consumer experience will mean significant alteration in the way care is organized, financed, and delivered and will likely unfold over the next decade and a half.
