Une innovation pour un meilleur continuum de soins en oncologie : développement et implantation d'un portail web d'échange des savoirs

Despite numerous initiatives across Quebec to improve the management of discharge planning and the transition of care provided to patients with cancer, several challenges remain, notably with regard to the transfer of care, follow-up, support and communication between health professionals. This article presents the protocol of a research project aiming to respond to these challenges through the setting up and assessment of an innovative clinical tool which uses cutting-edge technology, the Knowledge Exchange Portal (KEP). The KEP brings together, in a new interactive space, nurses, researchers and other partner professionals and patients, to share their knowledge (tacit, scientific) and improve the continuum of care in oncology. This study adopts a participative approach and is carried out in six clinical environments covering the care continuum, from prevention to palliative care. It aims to help improve discharge planning and care transitions through the use of knowledge and the updating of clinical practices.
