Nutrition artificielle après court circuit gastrique

RésuméLors de complication fistuleuse après une chirurgie bariatrique, il est nécessaire d’avoir recours à la nutrition artificielle pour nourrir le patient et lutter contre une dénutrition défavorable pour la poursuite de la prise en charge médicale. Le cas présenté ici permet de réfléchir aux voies d’abord à utiliser pour nourrir le patient, en fonction du contexte clinique chirurgical. Il s’agit d’un cas de complication fistuleuse au niveau de l’anastomose gastrojéjunale, qui représente la principale complication postopératoire.

In case of fistula secondary to Roux-en-Y gastric bypass surgery, the nutritional care, including nutrition support is mandatory to decrease the risk of malnutrition, the latter being related to unfavorable outcome (delayed healing and increased risk of nosocomial infection). The present case report allows discussing the different feeding routes in relation to the surgical situation, in order to improve the nutritional status. This article reports the case of a gastrojejunal anastomosis fistula, which is the main postoperative complication after Roux-en-Y gastric bypass.
