Introduction. Chronic rupture is a rare complication of AAA (abdominal aortic aneurysm). The clinical course is insidious and with no haemodynamic repercussions, which means it may be treated surgically. Clinical case. A 73 year-old male with chronic lumbar pain was diagnosed by chance with an AAA and a chronic retroperitoneal haematoma, which had caused severe osteolysis in the L3-L5 vertebral bodies. Surgical treatment was scheduled, with a symptomatic intrarenal fistula appearing in the post-operative period, which was resolved with selective embolisation. Conclusion. The contained aortic aneurysm rupture should be considered as a rare cause in the differential diagnosis of chronic lumbar pain in patients of advanced age. [ANGIOLOGIA 2009; 61: 355-8]
