Coronary cameral fistula in an asymptomatic young female presenting with dynamic ST-T changes

Coronary cameral fistulas are congenital abnormal communications between the epicardial coronary arteries and the cardiac chambers. These fistulas can be acquired in some cases. These fistulas can produce inducible ischemia as a result of shunting of blood into the cavity of heart bypassing the myocardial capillary bed and producing coronary steal phenomenon. These patients may present with angina, exertional breathlessness, congestive cardiac failure or can be asymptomatic. We present a case of a 25-year-old, underweight female, who presented with very high blood sugar values and dynamic ST-T wave changes in routine electrocardiogram, and who was later found to have a coronary cameral fistula by coronary angiography.
