Nouvelles perspectives en endoscopie thérapeutique : NOTES

RésuméLa natural orifice translumenal endoscopic surgery (NOTES) (ou chirurgie endoscopique transluminale par les orifices naturels) consiste à aborder la cavité péritonéale à l’aide d’un endoscope souple, à travers les orifices naturels puis à travers la paroi du tube digestif ou des voies urogénitales. La NOTES peut être pure, ou éventuellement combinée à la laparoscopie (mode hybride). La NOTES permet la réalisation d’interventions sans incision cutanéomusculaire de la paroi abdominale (avantage esthétique, absence de complication de paroi et diminution potentielle des douleurs pariétales postopératoires). La NOTES pourrait bénéficier plus particulièrement aux patients obèses ou en situation de réanimation ou de soins palliatifs. Parmi les réalisations sur animal, on peut citer la péritonéoscopie avec biopsies, l’hystérectomie, l’ovariectomie, la ligature de trompes, la gastrojéjuno-anastomose, la cholécystectomie, la splénectomie, la néphrectomie, la cure de hernie de paroi, aussi bien par voie transgastrique que par voie transpelvienne. En clinique humaine, l’abord transvaginal est préféré. La péritonéoscopie, l’appendicectomie et la cholécystectomie sont les principales indications développées. On attend également de cette recherche des innovations en termes d’instrumentation endoscopique (possibilités nouvelles de suture, anastomose, traction, triangulation). La NOTES est susceptible de faire évoluer à la fois l’endoscopie interventionnelle (en autorisant davantage d’interventions transmurales et en faisant progresser la suture endoscopique) et la chirurgie minimale invasive (en intégrant les endoscopes et instruments souples à l’arsenal laparoscopique).

NOTES can be combined to laparoscopic surgery in so-called « hybrid » techniques. Indirect benefits are expected from this research, with possible technological innovations in the field of endoscopic instrumentation (including sutures, anastomosis, traction and triangulation). Overall, NOTES is believed to make evolve both interventional endoscopy and minimally invasive surgery.
