Le kyste lymphoépithélial du pancréas : à propos d’un nouveau cas

RésuméNous rapportons un cas de kyste lymphoépithélial du pancréas découvert fortuitement à l’imagerie chez un homme de 54 ans. Le scanner objectivait une masse kystique polylobée hypodense de 10 cm centrée sur l’isthme pancréatique. La cytoponction n’était pas contributive. En l’absence de diagnostic préopératoire avec notamment une incertitude concernant la bénignité, il a été réalisé une duodénopancréatectomie céphalique.Le kyste lymphoépithélial du pancréas est une lésion bénigne rare de diagnostic préopératoire difficile. Histologiquement, les parois kystiques sont bordées par un épithélium malpighien kératinisant reposant sur un tissu lymphoïde. Les cavités kystiques contiennent des amas de kératine inconstamment visualisés en imagerie. L’analyse cytologique du produit de ponction et histologique des microbiopsies préopératoires, si elles sont contributives, peuvent permettre d’éviter une chirurgie lourde.

Histologically, the cyst wall is lined by mature keratinizing squamous epithelium and a distinct surrounding lymphoid tissue layer. The cysts are filled with keratin plugs that are not always visualized on imaging. Cytological and histological analysis of fine-needle aspiration material if the sample material is sufficient may help avoid extensive surgery.
