RésuméLa description et l’étude des réactions croisées entre allergènes, en particulier entre les pollens et les aliments végétaux, a commencé autour des années 1970–1980. Après la découverte des premiers syndromes de sensibilisations et d’allergies croisées, un grand nombre de nouvelles entités cliniques ont été décrites. Si cet important chapitre de l’allergologie concerne surtout les réactions croisées entre pollens et aliments végétaux, des situations de ce type ont également été décrites entre les allergènes et les aliments d’origine animale. Les auteurs envisagent les principales réactions croisées entre pollens et fruits et légumes, d’une part, puis entre les allergènes et les aliments d’origine animale, d’autre part. Cette revue se termine par des implications pratiques cliniques et thérapeutiques : valeur de l’évaluation clinique, apport diagnostique et thérapeutique des allergènes de recombinaison, importance des tests de provocation aux aliments par voie orale, prédiction du risque allergique, place de l’immunothérapie spécifique dans la prise en charge d’une allergie croisée.

The description and study of cross-reactions between allergens, pollen and food of plant origin in particular, began around 1970–1980. After the discovery of the first sensitization syndromes and cross-reacting allergies, a large number of new clinical entities were described. While this important chapter of allergy concerns, above all, cross-reactions between pollen and food of plant origin, cases of this type were also described between allergens and food of animal origin. The authors will first consider the principal cross-reactions between pollen and fruit and vegetables, and then consider those between allergens and food of animal origin. This review will end by considering the practical clinical and therapeutic implications of cross-reactions: the value of clinical evaluation, the diagnostic and therapeutic contribution of recombinant allergens, the importance of oral provocation tests with food, the prediction of allergic risk, the place of specific immunotherapy in the management of a cross-reacting allergy.
