The present article reviews the way in which NSAID should be used. A literature search yielded only two studies with very different characteristics that suggest a slight modifier effect (delay in radiographic progression) with continuous NSAID use. Although current data suggest a possible impact of NSAIDs on disease progression, there is insufficient evidence to recommend prolonged NSAID use in patients with AS, especially if the possible side effects and the costs of their use are taken into account. It is our recommendation that NSAIDs be administered as symptomatic treatment and that the dose and duration of NSAID therapy be individually tailored, without necessarily prolonging their use.
