Expectativas, preferencias y satisfacción de los pacientes con artritis reumatoide en tratamiento con infliximab

Fundamento y objetivoEl objetivo del estudio ha sido evaluar las expectativas, preferencias y satisfacción de los pacientes con artritis reumatoide (AR) respecto al tratamiento con infliximab, así como la relación con la calidad de vida relacionada con la salud, en situación de práctica clínica habitual.Pacientes y métodoParticiparon 198 pacientes con AR que iniciaban tratamiento con infliximab al comenzar el estudio. Se evaluaron las expectativas, preferencias, satisfacción, calidad de vida relacionada con la salud y evolución clínica de los pacientes, y la seguridad del tratamiento a las 2, 6 y 14 semanas, coincidiendo con las infusiones de infliximab.ResultadosMás del 85% de los pacientes prefirió tratarse en el hospital, al estimar positivo el hecho de estar en contacto con otros pacientes, enfermeras y médicos, y poder hablar de su enfermedad. Entre el 70 y el 80% se mostraron satisfechos con los resultados del tratamiento con infliximab. A las 2 semanas de seguimiento se había reducido el número de articulaciones tumefactas (70%) y de articulaciones dolorosas (75%). A las 14 semanas, el 56,6% cumplía criterios ACR20, el 31,5% ACR50 y el 11,3% ACR70. El 33,8% presentó acontecimientos adversos. Las puntuaciones del EuroQoL-5D mejoraron, hasta aproximarse a la de la población general. La capacidad funcional mejoró de forma significativa a las 2 semanas de tratamiento en más del 50% de los pacientes, y en más del 70% a las 14 semanas.ConclusionesLos resultados obtenidos muestran las preferencias de los pacientes por el tratamiento hospitalario en lugar del tratamiento domiciliario, así como un alto grado de satisfacción con el infliximab.

Background and objectiveOur goal was to assess expectations, preferences and treatment satisfaction in patients suffering from rheumatoid arthritis (RA) treated with infliximab and their relationship with health related quality of life in real clinical practice.Patients and method198 patients with AR participated in the study who started medication with infliximab at the beginning of the survey. Evaluation of expectations, preferences, satisfaction, health related quality of life, clinic evolution of patients and safety were made 2, 6, and 14 weeks after, coinciding with infliximab transfusions.ResultsMore than 85% of the patients preferred to be treated in the hospital. They valued positively to be in contact with other patients, nurses and doctors in order to speak about their illness.Between 70% and 80% of the patients were satisfied with the infliximab treatment. After 2 weeks patients had reduced tender joint count by 70% and swollen joint count by 75%. At the end of the 14th week, 56.6% of the patients matched criteria ACR20, 31.5% ACR50 and 11.3% ACR70. 33.8% of patients had adverse events. The results in the EuroQol-5D indicated that patients improved their punctuations getting closer to those of the general population. Functional capacity of 50% of the patients improved significantly after the first 2 weeks of treatment and after 14 weeks this percentage reached the 70%.ConclusionsThe results demonstrate that patients preferred to be treated in the hospital rather than in their houses. Also, patients reported a high level of satisfaction with infliximab.
