Nefropatías tubulointersticiales

ResumenLas nefropatías tubulointersticiales (NTI) engloban aquellas enfermedades que afectan al intersticio y a las células del túbulo renal. Se caracterizan por la presencia de un infiltrado inflamatorio celular en dichos compartimentos. Se clasifican en agudas y crónicas y su etiología puede ser secundaria a fármacos, infecciones o enfermedades autoinmunes/sistémicas. Es típica la presencia de insuficiencia renal junto con piuria, cilindros leucocitarios y eosinofiluria. La biopsia renal es la prueba gold standard. En las formas agudas un posible tratamiento son los esteroides. Se recomienda el inicio precoz del tratamiento aun careciendo de pruebas histológicas. La mayoría de los casos de nefritis tubulointersticial aguda (NTIA) por fármacos mejora espontáneamente tras la suspensión del causante, aunque la función renal puede no recuperarse ad integrum. En la NTIA por infecciones u otras causas, el tratamiento es etiológico.

Tubulointerstitial nephritis (TIN) are characterized by interstitial infiltrates and tubulitis. They are classified into acute and chronic and its etiology may be secondary to drugs, infections or autoimmune / systemic diseases. They classically present renal failure associated with pyuria, eosinophiluria, and cell cast. Renal biopsy is the gold standard test. Therapy with glucocorticoids may be considered in acute damage. Early initiation of treatment is recommended even without histological evidence. Most cases of acute tubulointerstitial nephritis (AIN) due to drugs improve after the potentially offending agent is discontinued, although kidney function may not return ad integrum. The treatment of AIN due to infections or other causes, should depend on etiology.
