Hepatitis crónica por virus B. Historia natural y tratamiento

ResumenLa infección crónica por el virus de la hepatitis B (VHB) constituye un importante problema de salud a nivel mundial por su amplia distribución y elevada prevalencia.Tras la infección aguda, un porcentaje variable de pacientes según la edad de exposición al virus, desarrollará una infección crónica, cuya historia natural evoluciona en fases que no siempre se suceden de forma consecutiva (inmunotolerancia, inmunorreactividad, seroconversión y negativización del antígeno de superficie).Actualmente se han producido grandes avances en el tratamiento de la hepatitis crónica B gracias a la aparición de fármacos muy potentes y con una elevada barrera genética frente al desarrollo de resistencias (tenofovir y entecavir). No obstante, el interferón pegilado sigue siendo una opción válida en determinados grupos de pacientes.

Chronic hepatitis B (HBC) is a major health problem worldwide for its wide distribution and high prevalence. Following acute infection, a variable percentage of patients according to age of exposure to the virus, develop chronic infection, whose natural history evolves in phases (immune tolerance, immunoreactivity, seroconversion and lost of HBsAg) that not always presented consecutively. Currently there have been major advances in the treatment of chronic hepatitis B thanks to more efficacious drugs that also have higher genetic barrier to resistance (tenofovir and entecavir). However, pegylated interferon remains a valid option in certain patients.
