Protocolo de actuación ante el paciente agitado

ResumenEl paciente agitado es un síndrome de difícil abordaje inicial por la falta de cooperación por parte del paciente. La orientación diagnóstica inicial es de vital importancia para descartar organicidad, ya que la gravedad puede oscilar desde la banalidad hasta el riesgo vital del paciente, del personal sanitario y de seguridad. Es una urgencia médica frecuente tanto a nivel intrahospitalario como extrahospitalario. Las medidas terapéuticas se han de instaurar de forma precoz y con un orden establecido de prioridad para poder abordar satisfactoriamente la situación médica. Los neurolépticos típicos y atípicos son de elección en la agitación orgánica y en la psiquiátrica. Las benzodiacepinas se pueden asociar o no en la agitación de causa psiquiátrica según la gravedad. Existen pacientes que no cumplen criterios de organicidad ni de patología psiquiátrica, denominándose agitación situacional o reactiva.

The agitated patient is a syndrome of difficult initial approach by lack of cooperation from the patient. It is a common medical emergency both inpatient and outpatient level. The therapeutic measures have to be initiated early and with an established order of priority to address the health situation successfully. Typical and atypical neuroleptics are of choice and the organic psychiatric agitation. Benzodiazepines may be associated or not cause psychiatric agitation by gravity. There are patients who do not meet criteria organicity or psychiatric pathology being called situational or reactive agitation.
