Valvulopatía mitral y tricuspídea

ResumenLa valvulopatía mitral ha cambiado en los últimos años su epidemiología y tratamiento de forma significativa. La insuficiencia mitral constituye la segunda causa más frecuente de valvulopatía en el mundo occidental, siendo la etiología degenerativa y la isquémica las más frecuentes. La estenosis mitral ha disminuido de forma significativa su incidencia en el mundo occidental, siendo la etiología reumática la más frecuente. El ecocardiograma es la técnica de imagen de elección para el diagnóstico y seguimiento de ambas entidades. La cirugía sigue siendo la forma más frecuente de tratamiento de la valvulopatía mitral, aunque se ha producido un aumento en la tendencia a la reparación de la válvula mitral en relación con la insuficiencia fundamentalmente. Además, sigue teniendo un papel relevante la valvuloplastia percutánea en aquellos enfermos subsidiarios a la misma. Se han desarrollado técnicas de tratamiento percutáneo mediante clip mitral para la insuficiencia mitral, aunque con indicaciones muy restringidas en la actualidad.

Mitral valve disease epidemiology and treatment have changed significantly in recent years. Mitral valve insufficiency is the second leading cause of valvulopathy in the Western world. The most common etiologies are degenerative and ischemic disease. In the Western world, mitral stenosis has become less common and the most cases are rheumatic in origin. Echocardiogram is imaging technique of choice in the diagnosis and follow-up of both entities. Surgery remains the most common treatment modality for mitral valve disease although there has been an increasing trend towards mitral valve repair. Percutaneous mitral valvuloplasty continues playing an important role in the management of subsidiary patients for receiving treatment. Percutaneous mitral valve repair using the MitraClip system has been developed although currently its indications are very restricted.
