Protocolo diagnóstico y terapéutico de las vaginitis infecciosas: vulvovaginitis candidiásica, tricomoniasis y vaginosis bacteriana

ResumenLas vaginitis infecciosas se producen por la alteración de la flora habitual de la mucosa vaginal. Los síntomas más frecuentes son prurito vaginal, síndrome irritativo miccional y aumento del flujo vaginal o leucorrea, siendo este último el síntoma principal. Existen causas fisiológicas que pueden aumentar el volumen del flujo vaginal, así la vaginitis atrófica en mujeres postmenopáusicas es una causa frecuente de leucorrea. El diagnóstico definitivo se conseguirá mediante el examen en fresco, la tinción de Gram y el cultivo del exudado vaginal. Debe realizarse tratamiento dirigido a la entidad causal, en ningún caso se realizará tratamiento empírico en función de los síntomas.

Infectious vaginitis are caused by the alteration of the normal flora of the vaginal mucosa. The most common symptoms are vaginal itching, irritative voiding syndrome and the increase of the vaginal discharge, being this last symptom the main one. There are physiological reasons that can increase the volume of vaginal discharge, and atrophic vaginitis in postmenopausal women is a common cause of vaginal discharge. The definitive diagnosis will be achieved by the direct smear, the Gram stain and the culture of the vaginal discharge. It must be performed a treatment directed toward the causative agent, in any case empirical treatment may be based on symptoms
