Hipercolesterolemias genéticas

ResumenLas hipercolesterolemias genéticas son un grupo muy heterogéneo de trastornos con transmisión, penetrancia y expresividad variables pero que se acompañan de elevado riesgo cardiovascular. Consideramos hipercolesterolemia genética cuando la concentración sanguínea de colesterol total o colesterol LDL son superiores al percentil 90 de la población en ausencia de una causa secundaria. Aproximadamente el 50% de los supervivientes de un infarto prematuro tienen alguna forma de hipercolesterolemia genética.Su diagnóstico precoz es importante porque tienen una estrategia terapéutica propia; facilita la búsqueda de nuevos casos entre familiares, la adhesión al tratamiento y resulta coste-efectivo.Repasamos las diferentes formas de hipercolesterolemias con especial hincapié en la hipercolesterolemia familiar monogénica por mutaciones en el gen que codifica el receptor LDL.

Genetic hypercholesterolemias comprise a heterogeneous group of genetically transmitted disorders with variable penetrance and expressivity and associated with increased risk of cardiovascular disease. Genetic hypercholesterolemia is defined by blood levels of total cholesterol and LDL cholesterol (low density lipoprotein cholesterol) higher than the 90th percentile of the total population in the absence of secondary cause. Approximately 50% survivors of premature myocardial infarction have some genetic forms of hypercholesterolemia. Early identification of persons with genetic hypercholesterolemias and their relatives, and early start of the therapeutic strategy itself are essential and improve therapeutic adherence and its cost-effectiveness. Different forms of hypercholesterolemia, particularly monogenic familial hypercholesterolemia due to mutations in the LDL-receptor gene are reviewed in this paper.
