Points essentielsLe traitement médicamenteux de la cardiomyopathie hypertrophique est le traitement de première intention mis en jeu dès l’apparition des signes cliniques.Les bêtabloquants sont proposés pour les patients symptomatiques sans gradient ou avec un gradient n’apparaissant qu’à l’effort.Le vérapamil est utilisé classiquement en deuxième intention quand les bêtabloquants sont insuffisants ou inefficaces et doit être évité quand il existe une obstruction de repos avec des symptômes sévères.Le disopyramide est indiqué pour les patients avec obstruction et qui n’ont pas répondu aux bêtabloquants et/ou vérapamil et un traitement par antivitamine K est largement prescrit car il s’agit d’une pathologie très emboligène.
