Should this treatment fail, a repeated repair or a sphincter replacement (graciloplasty or artificial sphincter) is called for.Neuromodulation therapy is often indicated in the absence of obvious rupture, especially in cases of pudendal neuropathy.

Dès qu’un patient consulte pour cette affection, l’interrogatoire et l’examen clinique doivent permettre d’en évaluer la cause et la sévérité.Deux examens essentiels et atraumatiques sont des outils d’aide à la décision : l’échographie endoanale et la manométrie.Le traitement médical doit toujours être proposé.Dans quelques cas, une intervention chirurgicale est nécessaire : une rupture sphinctérienne externe évidente, éventuellement confirmée par une échographie endoanale, conduit à une réparation directe. En cas d’échec, il faut proposer une réparation itérative ou une substitution sphinctérienne (graciloplastie ou sphincter artificiel).La neuromodulation tire ses indications de l’absence de rupture évidente : il s’agit alors le plus souvent d’une neuropathie pudendale.
