RésuméLe vieillissement de la population fait que le nombre de patients âgés porteurs de cancer de la prostate s’accroit d’année en année. Parmi les options de traitement curatif chez un sujet âgé, la radiothérapie externe est l’option la plus souvent choisie. Un traitement combiné comportant une radiothérapie et une hormonothérapie doit être privilégié par rapport à un traitement hormonal seul y compris chez les patients âgés dès que l’espérance de vie dépasse 4 à 5 ans. Il convient de définir les indications de cette radiothérapie en évitant les risques de sur et sous traitement, d’adapter les modalités du traitement à l’âge en évaluant sa toxicité potentielle et de discuter des alternatives possibles. En cas de cancer localisé de la prostate chez des hommes qui ont un vieillissement harmonieux, un traitement standard sera proposé qui privilégiera la radiothérapie éventuellement associée à l’hormonothérapie en cas de facteurs pronostiques péjoratifs.
