Prise en charge du cancer rénal métastatique

RésuméLe cancer du rein métastatique concerne 30 à 50 % des patients. Les principales localizations sont les poumons, le foie, les os et le cerveau. En cas de localisation unique, le traitement est avant tout chirurgical quel que soit l’organe atteint avec des rémissions parfois longues. En situation plurimétastatique, le traitement fait appel désormais aux antiangiogéniques en 1re et en 2e ligne. En France, aujourd’hui quatre molécules ciblées ont une AMM (sunitinib, sorafenib, temsirolimus, bevacizumab). Les associations de molécules sont en essai pour tenter d’améliorer le pronostic des patients qui reste péjoratif. Enfin la néphrectomie reste indiquée dans l’attente d’études prospectives sur son réel impact.
