Neuro-spect: Imaginología funcional en psiqyuiatría

ResumenDurante los últimos años la imagenología funcional presentada por Neuro-Spect se ha ido incorporando progresivamente al diagnóstico diferencial de la patología psiquiátrica especialmente trastorno del ánimo, psicosis, esquizofrenia, trastornos cognitivos como enfermedad de Alzheimer´s, el trastorno obsesivo compulsivo, el déficit atencional y sobretodo los efectos cerebrales de neurotóxicos. En este trabajo publicaremos y comentaremos inicialmente los hallazgos en infarto cerebral hiper-agudo durante y post-tratamiento mediante trombolisis arterial como una forma de demostrar la sensibilidad y especificidad de Neuro-Spect en el trabajo clínico. Sobre metodología comentaremos especialmente que efectuamos una doble reconstrucción de las imágenes una sin corrección de atenuación para las imágenes laterales, anterior y posterior y la imagen superior de la corteza cerebral y una segunda reconstrucción con corrección de atenuación con un coeficiente de Chang de 0.1 que aplicamos a las imágenes parasagitales a la imagen inferior del cerebro en el cual además removemos el cerebelo con el objeto de poder inspeccionar con facilidad los aspectos inferiores de los lóbulos occipitales y temporales, igualmente está modalidad se emplea para el estudio de los ganglios basales.El software que empleamos en el software Oasis de la compañía Segami Corp. EE.UU.Las imágenes son adquiridas con un sistema E.CAM de dos cabezales Siemens Co y los resultados son comparados con una base de datos normales para el mismo grupo etario y los resultados entonces demostrados con un escala de colores calibrada en desviaciones estándar sobre y bajo el rango normal que aparece en color gris.

SummaryDuring the last years Functional Brain Imaging, represented by NeuroSPECT is becoming progressively important in the differential diagnosis of clinical psychiatry. This is particularly important in mood disorders, Psychosis, schizophrenia, Alzheimer’s Disease, Obsessive Compulsive Disorder, Attention Deficit Disorder and very importantly in the study of neurotoxicity of substance abuse. In this paper we will describe the findings in hyper acute stroke, before and post arterial thrombolisis, this done in order to demonstrate the sensitivity and specificity of neuroSPECT. On method we will comment that we make a double reconstruction of the images with and without attenuation correction by means of the method of Chang. The images without attenuation correction are projected in the anterior, posterior, both lateral and superior cortical images while the mesial images and the inferior cerebral image are projected with attenuation correction. The software we use is produced by Segami Corp in USA.The images are acquired after HMPAO injection and compared against a normal age matched data base and the results are demonstrated with a color scale calibrated in standard deviations above and below the normal mean for the same normal age group. The normal range appears in the color gray.
