The lower urinary tract symptoms in men have traditionally been interpreted as secondary to lower urinary tract obstrution caused by prostate enlargement and called “prostatism”. This is an inaccurate simplification since there are other obstructive diseases and non-obstructive urinary disturbances, that may have the same symptoms, but differ in their physiopathology and therapeutic management. An accurate diagnosis, will not only prevent deterioration of the quality of life of these patients but also avoid complications arising from inadequate management. The aim of this article is to provide tools to interpret the lower urinary tract symptoms in men in relation to the physiology and physiopathology of the vesico sphincter complex, diagnose their main causes and outline treatment options in a rational way.
