Tratamiento médico de tumores hipofisarios

ResumenSe presentan los diferentes tipos de tumores hipofisiarios y para cada uno las opciones de tratamiento médico disponibles actualmente. Se detallan sus indicaciones, los resultados que se estima posibles, sus efectos secundarios y contraindicaciones.Se señala claramente las situaciones en que el tratamiento quirúrgico no requiere ser la primera opción.Para los Prolactinomas se analizan las opciones vigentes: bromocriptina y cabergolina.Para los tumores secretores de GH las drogas disponibles son la Somatostatina y Pegvisomant.Para los tumores productores de ACTH se discute el rol del Ketoconazol y Pasireotide.Se hace un breve análisis de los tumores antes llamados “no funcionantes” y que actualmente se sabe producen TSH, FSH y/o LH y aquellas drogas susceptibles de ser utilizadas.

SummaryThe different types of pituitary neoplasms are pointed out and for each of them the drugs available at present are discussed. The results expected with them are analyzed as well as their secondary effects and contraindications.Clearly mentioned are the situations in which the surgical removal of the tumor is not required as first choice.For Prolactinomas the medications discussed are bromocriptine and cabergoline.For GH producing tumors the medications Somatostatin and Pegvisomant are analyzed.The role of Ketoconazole and Pasireotide in ACTH producing hypophiseal adenomas is discussed.Lastly a brief review of the so called “non functioning” adenomas now known to produce TSH, FSH and/or LH and the medications available to treat them is presented.
